CLOCK HOURS OF TRAINING
FOR
HOME CARE
Child care professionals who offer home care may earn clock hours of training using the courses from
Self Dimensions, Inc. Please check with your individual
state requirements to see whether or not our materials are acceptable in your state.
Realizing that the cost of purchasing a video tape for training in the home might be prohibitive, Self Dimensions has established a rental program for home care providers.
If you wish to earn your clock hours of training using the self-study programs from Self Dimensions, Inc., please do the following:

Once you have selected the course(s), complete the
ORDER FORM. Include your check, money order or credit card information
(credit card orders may be faxed to 1-806-792-9309) and mail to:

Courses will be mailed (*UPS or US Post Office) within 48 hours.

Your course includes written material to be completed along with a 30 day rental of the video tape. The tape must be returned to Self Dimensions at the end of the 30 days.

When you have completed the written course work, return it to Self Dimensions (Attention: Home Care Grading). Upon successful completion of the course work, a certificate showing the number of clock hours earned, along with the graded course work will be sent to you, usually within three days.

Although we keep a copy of the test and the certificate on file, it is the responsibility of the individual to retain the original certificate documenting the successful completion of the course.
The course rental fee is based on $4.00 per clock hour awarded
and includes a video tape and the written course work.
